DESCRIPTION
A not-for-profit, non-sectarian organization founded in 1980.
Care is provided to patients and their families in Mercer, Atlantic,
Burlington, Camden and Gloucester counties regardless of
insurance coverage or ability to pay for services. Care is
available to patients with any end-stage illness when life
expectancy is six months or less. Hospice patients seek
palliative (or comfort) care for pain control and symptom
management rather than curative treatments. Patients and their
families receive the care they need in the comfort of their home,
in long-term care settings, or in the hospital. The hospice team
includes a full-time Medical Director, Registered Nurses,
Licensed Practical Nurses, Certified Home Health Aides, Social
Workers, Bereavement and Spiritual Support Counselors and
specially trained volunteers. Each patient is assigned a primary
Registered Nurse who works with the patient's own physician to
develop a personal plan of care.
PROGRAMS
Samaritan Hospice Care: Provides specialized health care and
supportive services for people facing serious, progressive
illness. Samaritan's team includes nurses, certified home health
aides and social workers, as well as grief and spiritual
counselors. They provide patient care and supportive family
services in the comfort of home, in long-term care settings, or
in the hospital. The patient's own physician directs the plan of
care, outlining the services of Samaritan's staff and may include
medications, medical equipment and supplies. Samaritan's
services are covered by Medicare, Medicaid, HMOs and many
other commercial insurance plans. A special Veteran's Hospice
Outreach program team undergoes special training to address
the unique physical and emotional concerns common among
veterans of different wars and conflicts.
SOLO Hospice Care: SOLO services enable patients to stay
safely in the comfort of home and maintain their independence
for as long as possible. These services are helpful for people
who live alone, spend much of their time alone when family or
caregivers are working or away from home, or people whose
family or caregivers have their own limitations.
Transitions: Transitions is a volunteer support service which
offers reassurance and support for people facing life-
threatening illness. Transitions volunteers provide assistance
for people who feel they can benefit from a helping hand,
offering friendship and help in a variety of tasks, such as just
listening or offering rides to markets or doctor's offices. The
program is offered at no charge by Samaritan as a service to our community.

Offers counseling and support -to assist people when a loved
one is seriously ill, or has died. Services include individual
counseling, family counseling, bereavement support groups for
children and adults, consultation and education.
Living with Heart Failure: Provides expert comfort care, symptom
management, education and services that address the unique
circumstances, such as rapid decline and recovery, that are
signatures of this progressive condition.
Pediatric Palliative Care: a specialized pediatric palliative care team
provides medical, emotional and spiritual support for families coping
with a child's life-limiting illness. The team works to reduce a child's
pain and symptoms, as well as facilitate communication and help
coordinate community resources.
