DESCRIPTION
The state regulatory government agency administered by the
New Jersey Department of Health and Senior Services which
oversees the licensing of long-term care facilities, such as
nursing homes, assisted living, comprehensive personal care,
and adult and pediatric day health services. The division also
oversees the licensing of acute care services, such as acute care,
rehabilitation, private psychiatric, and long-term acute care
hospitals; ambulatory facilities such as dialysis centers,
ambulatory surgery centers, home health and hospice agencies,
primary care providers, family planning centers, federally
qualified health centers, and orthotripsy/lithotripsy provides,
and a variety of diagnostic service centers such as sleep centers,
CT and MRI, radiology oncology and PET scanners.

Assessment and Survey Program: Program which provides
surveys and complaint response at licensed facilities. Call 609-
