DESCRIPTION
An agency which specializes in assisting older adults and their families with long term care arrangements and in home care. Senior Care Management is a licensed health service firm.

SERVICES
Geriatric care long term planning, home health aides, care management services.

PROGRAMS
Case Management: Provides 1) an in-home assessment of the older adult's needs and the family's concerns; 2) recommends a care plan of appropriate services and supports; 3) arranges and coordinates the services, referrals, and assistance with government programs when necessary; 4) continues monitoring of the older adult and intervenes when circumstances change; and 5) acts as liaison and support for family members who live at a distance.

Home Care: Provides home health aids supervised by an R.N. for personal care services, medication supervision, meal preparation and assistance with household tasks; and assistance with errands, appointments, social activities and household tasks.

Court Appointed Guardians and Conservators: Serves as guardian for older adults who have been deemed incompetent by the court.
